The Records Checklist

What to gather before your child's next specialist appointment

Specialists ask the same questions in the same order. Gather these and the appointment is about your child, not the paperwork.

    • Milestones gained and lost, with approximate dates
    • A one-paragraph timeline, from the first worry to today
  2. Evaluations

    • Early Intervention or school evaluations
    • Developmental or autism evaluations
    • Hearing and vision tests
    • Any psychology or neuropsychology reports
  3. Medical records

    • Pediatrician growth charts
    • Hospital or ER visits
    • Imaging reports
    • EEG reports
    • Any genetic testing already done, with the actual result pages
  4. Medications and supplements

    • Current and past, with doses
    • What helped, and what didn't
    • Side effects
  5. Family history

    • Seizures
